Mission Statement/Our Reason for Existence:

To provide the highest quality of physical therapy treatments and promote wellness  among community dwelling adults 55 years and older. We recognize the unique needs of the older adult population and are committed to providing highly specialized, individualized care.

Who can benefit from physical therapy?

People who are struggling with any of the following pathologies and/or impairments:

  • Post Joint Replacement/ Hip/Knee/Shoulder
  • Pre & post-operative orthopedic injuries
  • Spinal pathologies
  • Cardiac/Pulmonary pathologies
  • Balance Difficulty
  • Muscle Weakness
  • Osteoarthritis
  • Osteoporosis
  • Stroke/CVA
  • Functional Decline/Debility
  • Vestibular Disorders and Dizziness
  • Parkinson’s Disease
  • Restricted joint motion
  • Poor endurance or fitness level
  • Overuse injuries
  • Poor soft tissue flexibility/elasticity
  • Muscle/Joint inflammations

What to Expect:

With a prescription from your doctor, a licensed physical therapist (PT) will perform a comprehensive evaluation where we will meet with you and your family to get to know you and understand how your problem is affecting your life.  During this evaluation, your therapist will establish an individualized plan of care and expected goals agreed upon by you and your therapist.  You can expect each treatment to be 1-on-1 with your therapist for an entire hour.  Treatment duration is different for each individual, but is generally 2-3 visits/week for 1 month.  Re-evaluations are performed every 30 days until the patient has met all of his/her goals.  Upon discharge, each patient has the option of joining our Aftercare program.

Aftercare Program

Unassisted Aftercare

Individuals may utilize the equipment in the therapy facility to carry out the exercise program, as established by the physical therapist.  This does not include direct supervision or assistance and may be completed at the convenience of the individual during the hours of 8am to 12pm and 1pm and 5pm M-F (no schedule required).  Therapy staff is always available answer questions that you may have regarding the exercise program or equipment.  This program requires a pre-payment of $25 per month with no limit to the times that you participate.

Assisted Aftercare

Individuals participate in scheduled sessions with a trained staff member, not a licensed therapist. The staff will assist or direct you through the exercise program that was established by your physical therapist.  Your program may be adapted at the discretion of the trained staff member after consultation with the therapist.  This program requires a pre-payment of the $240 for 12 scheduled sessions, usually 2-3 times per week.  Appointments must be established.

In order to qualify for either the Unassisted Aftercare or the Assisted Aftercare program, one must have received physical therapy at the Elder Care facility under a physician prescription.  Both programs allow you to continue to utilize the equipment in the physical therapy facility including but not limited to the air-resisted pneumatic machines, Nu-Step, Sci-Fit recumbent bicycle, treadmills, therabands, mat tables/plinths, various free weights, pulleys, stairs and more.
