What is Care Management?
Care Management is a professional service that helps you to live
with the problems of aging. The current system delivering health
and social services is often fragmented and confusing. Typically
there is not one place where all the information and services are
located. Care Management provides a service coordinator, advocate
and advisor who will help you maintain independence and enjoy safe,
comfortable living conditions in your home as long as possible.

Why should your family use Care Management services?
Intended to help both individuals and their caregivers, Care Management
can save you valuable time, expense, and worry. When a family situation
becomes overwhelming, and caregivers find their responsibilities
affecting their work, health, and personal lives, Care Management
services can offer guidance, develop a care plan, and supervise
an elderly person’s care. Not everyone knows the signs, symptoms,
and conditions of aging that are significant or need immediate attention.
Care Managers are trained to observe these signals.
How do Care Managers work?
Care Managers make an in-home visit to interview the client and/or
caregiver. Based on an evaluation of the individual’s mental
and physical capabilities, the home environment, finances, and social
support system, a care plan is developed with each client. The Care
Manager will work with various kinds of health care professionals
in the community to deliver the needed package of care. Follow-up
visits and phone calls are made to insure that the plan is working
well.
How long will a Care Manager be needed?
Care Management may be short-term care or long-term care depending
on the client’s condition and the family’s concerns,
and the support system available.
